Preventative Maintenance

I certify that the information given for credit retrieval purposes and credit analysis for purposes for extending credit is true and correct. By signing, I insure that I have the authority to sign on behalf of the Company/Applicant. My electronic signature below A) indicates my permission to allow S.M. Lawrence Company, Inc. to obtain credit information from the sources references and B) attests financial responsibility and willingness to pay invoices in accordance with terms. Accounts are due and payable net 30 days. After 30 days, accounts shall accrue a service charge of 1.5% per month until fully paid. If the account is turned over to a collection agency or attorney, Company/Applicant agrees to: (a) pay all accounts when due; (b) pay costs of collection and attorney fees if account is delinquent; and (c) all terms and conditions herein. I understand this application is being made in Davidson County. Tennessee and agree that Davidson County, Tennessee shall be the venue for any disputes involving same. I further acknowledge that I have read and fully understand the credit policy which constitutes the credit agreement.

Open account terms are extended to those customers who have submitted a completed credit application and have been approved for a line of credit. Terms are Net 30 days from invoice date for all sales, unless otherwise specified per contract. Accounts not paid within our terms may be placed on credit hold and/or subject to permanent suspension of credit privileges. The maximum allowable legal interest rate, plus a 1.5% finance charge per month, which is an annual percentage rate of 18%, will be charged to any invoices past 30 days.Those accounts that continually extend payments beyond the terms of sale will be subject to re-evaluation of their account status and line of credit limitations. Services, including labor and material, for customers with large or persistent overdue balances may be delayed or suspended until accounts are brought current. In the event that an account must be placed for collection, that account is responsible for all collection fees, expenses and legal costs.
